To receive the benefit, the member must submit the following:
Completed Claim Form
Copy of valid passport certified by a Notary Public or lawyer
Copy of Seaman’s Documentation Book (Fartsoppgave) or NIS Yellow Book (pages 1-3/5)
Copy of Original Membership card or copy of Membership Letter or Statement of Account
Copy of Medical Documentation from a doctor approved by the Norwegian Authorities, stating that the disability is permanent and renders you unfit for work (Certified by a doctor approved by Norwegian Authorities). The list of accredited doctors can be obtained from Storebrand or Fil-Nor GAIN, Inc.
Evidence that you have received compensation in accordance with the Norwegian Shipping Association’s Collective Bargaining Agreement.
