Death

The beneficiaries in case of death of a member are:

  • The legal spouse, if member has no spouse, then,
  • The children, if the member has no children then,
  • The parents
  • Person appointed by the member

To receive the benefit, the beneficiary/ies must submit the following:

  1. Completed Claim Form
  2. Copy of valid passport of seafarer certified by a Notary Public or lawyer
  3. Copy of valid passport of designated beneficiary certified by a Notary Public or lawyer
  4. Copy of Seaman’s Documentation Book (Fartsoppgave) or NIS Yellow Book (pages 1-3/5)
  5. Original Membership Card or copy of Membership Letter or Statement of Account
  6. Certificate of Employment issued by manning agency/ies or service records
  7. Copy of Marriage Contract, if married, certified by a Notary Public or lawyer
  8. Copy of Death Certificate certified by a Notary Public or lawyer
  9. Bank Account details containing bank account number, name and address of bank of the beneficiary only.
